Liability Waiver & Acknowledgment of Risk

Liability Waiver

I understand and agree that participating in any yoga, personal training, fitness class, workshop, assessment, coaching session, online or in-person training, or any other physical activity through Movefullife involves inherent risks, including but not limited to physical injury, strain, discomfort, illness, or even death. I acknowledge that my participation is voluntary and that I assume full responsibility for any risks, injuries, or damages that may occur to me or my property while engaging in Movefullife activities.

I further acknowledge that I am physically fit and have no medical conditions that would prevent me from safely participating in any activities offered by Movefullife. I understand that it is my responsibility to consult a physician prior to participating if I have any concerns about my health or physical ability.

I hereby waive, release, and hold harmless Movefullife, its owners, trainers, instructors, employees, contractors, volunteers, agents, and affiliates from any liability, claims, demands, or causes of action that may arise from my participation in Movefullife activities, whether caused by negligence or otherwise.

I also acknowledge that Movefullife does not provide medical advice and that all information, guidance, and training provided is for general educational and fitness purposes only.

I understand that I should be aware of my physical limitations and agree not to exceed them. If I am agreeing to this waiver for my child or legal dependent, I certify that I am the parent or legal guardian and have the authority to waive these rights on their behalf.

Photography & Media Release

I give full permission to Movefullife to use any photographs, videos, or recordings taken of me during sessions, classes, or events for marketing, social media, website, or promotional purposes, unless I notify Movefullife in writing that I do not consent to such use.

Payment Policy

I understand that all payments made to Movefullife are non-refundable and non-transferable unless otherwise stated in a written agreement. Tuition, subscription fees, or class fees must be paid before participation unless an alternative arrangement is agreed upon in writing by both parties.

If payment collection requires legal action, I agree to be responsible for all costs of collection, including attorney’s fees, court costs, and any applicable interest at the legal rate.

By submitting this form, I confirm that I have read, understood, and agree to all the terms above, including the Liability Waiver, Payment Policy, and Media Release.
